Base on cdc_ether, add the mii functions for RTL8152 and RTL8153.
The RTL8152 and RTL8153 support ECM mode which use the driver of
cdc_ether. Add the mii functions. Then, the basic PHY access is
possible.

Sparse reported an endianness bug in the assignment to hca_cap.uar_page_sz.
Fix the declaration of this field to be __be16 (which is what is in
the firmware spec), renaming the field to log_uar_pg_size to conform
to the spec, which fixes the endianness bug reported by sparse.

This patch is being created to use the UEFI bits in the type 219 SMBIOS
record in order to decide whether or not to execute BIOS code. This is a
better solution than to depend on the iCRU bit since not all future servers
will use iCRU.

In watchdog_ping(), 'start' is called automatically when 'ping' function call
is not configured.

The bits in BRIDGE_CAUSE are documented as RW0C - read, write 0 to
clear. If we read the register, mask off the watchdog bit, and
write it back, we're actually clearing every interrupt which wasn't
pending at the time we read the register - and that is racy.
Fix this to only write ~WATCHDOG_BIT to the register, which means
we write as zero only the watchdog bit.

The watchdog infrastructure in Dove is no different from that in
Orion5x or Kirkwood, so let's enable it for Dove. The only things
missing are a few register settings in Dove's bridge-regs.h.
Rather than duplicating the same register bit masks for the RSTOUTn_MASK
and BRIDGE_CAUSE registers, move the definitions into the watchdog
driver itself.

Interrupt request doesn't use the right API: The TWD watchdog uses a per-cpu
interrupt (usually interrupt #30), and the GIC configuration should flag it as
such. With this setup, request_irq() should fail, and the right API is
request_percpu_irq(), together with enable_percpu_irq()/disable_percpu_irq().
Nothing ensures the userspace ioctl() will end-up kicking the watchdog on the
right CPU.
There are no users of this driver since a long time and it makes more sense to
get rid of it as nobody is looking to fix it.
In case somebody wakes up after this has been removed and needs it, please
revert this driver and pick these updates (These were never pushed to mainline):

Pull thermal management updates from Zhang Rui:
"There are not too many changes this time, except two new platform
thermal drivers, ti-soc-thermal driver and x86_pkg_temp_thermal
driver, and a couple of small fixes.
Highlights:
- move the ti-soc-thermal driver out of the staging tree to the
thermal tree.
- introduce the x86_pkg_temp_thermal driver. This driver registers

A watchdog device may be stopped from userspace using WDIOC_SETOPTIONS
ioctl and flag WDIOS_DISABLECARD. If the device is closed after this
operation, watchdog_release() is called and status bits checked for
stopping it. Besides, if the device has not been unregistered a critical
message "watchdog did not stop!" is printed, although the ioctl may have
successfully stopped it already.
Without the patch a user application sample code like this will successfully
stop the watchdog, but the kernel will output the message
"watchdog did not stop!":

Commit 24778be "spi: convert drivers to use bits_per_word_mask" removed
what appeared to be redundant code from many drivers. However, it
appears that in the spi-bitbang case, these functions are required by
the spi-bitbang core, even if they don't do anything. Restore them.
For 3.12, the spi-bitbang core should be adjusted not to require these
callbacks to exist if they don't need to do anything.
This is the equivalent of Michal Simek's patch "spi/xilinx: Revert
master->setup function removal", applied to other affected drivers.

The efivars code requires EFI runtime services to function, so check
that they are enabled.
This fixes a crash when booting with the "noefi" kernel parameter, and
also when mixing kernel and firmware "bitness", e.g. 32-bit kernel with
64-bit firmware.

Userspace may produce vectors greater than MAX_SKB_FRAGS. When we try to
linearize parts of the skb to let the rest of iov to be fit in
the frags, we need count copylen into linear when calling macvtap_alloc_skb()
instead of partly counting it into data_len. Since this breaks
zerocopy_sg_from_iovec() since its inner counter assumes nr_frags should
be zero at beginning. This cause nr_frags to be increased wrongly without
setting the correct frags.
This bug were introduced from b92946e291
(macvtap: zerocopy: validate vectors before building skb).

Eric Dumazet fix the problem in dummy, but the ifb will occur the
same problem like the dummy modules.
Trying to "modprobe ifb numifbs=30000" triggers :
INFO: rcu_sched self-detected stall on CPU
After this splat, RTNL is locked and reboot is needed.
We must call cond_resched() to avoid this, even holding RTNL.

dm-switch is a new target that maps IO to underlying block devices
efficiently when there is a large number of fixed-sized address regions
but there is no simple pattern to allow for a compact mapping
representation such as dm-stripe.
Though we have developed this target for a specific storage device, Dell
EqualLogic, we have made an effort to keep it as general purpose as
possible in the hope that others may benefit.

This reorder actually improves performance by 20% (from 39.1s to 32.8s)
on x86-64 quad core Opteron.
I have no explanation for this, possibly it makes some other entries are
better cache-aligned.

This patch removes "io_lock" and "map_lock" in struct mapped_device and
"holders" in struct dm_table and replaces these mechanisms with
sleepable-rcu.
Previously, the code would call "dm_get_live_table" and "dm_table_put" to
get and release table. Now, the code is changed to call "dm_get_live_table"
and "dm_put_live_table". dm_get_live_table locks sleepable-rcu and
dm_put_live_table unlocks it.
dm_get_live_table_fast/dm_put_live_table_fast can be used instead of
dm_get_live_table/dm_put_live_table. These *_fast functions use
non-sleepable RCU, so the caller must not block between them.
If the code changes active or inactive dm table, it must call
dm_sync_table before destroying the old table.

This patch changes dm-bufio so that it submits write I/Os outside of the
lock. If the number of submitted buffers is greater than the number of
requests on the target queue, submit_bio blocks. We want to block outside
of the lock to improve latency of other threads that may need the lock.

Use __always_inline to avoid a link failure with gcc 4.6 on ARM.
gcc 4.7 is OK.
It creates a function block_div.part.8, it references __udivdi3 and
__umoddi3 and it is never called. The references to __udivdi3 and
__umoddi3 cause a link failure.

Use __GFP_HIGHMEM in __vmalloc.
Pages allocated with __vmalloc can be allocated in high memory that is not
directly mapped to kernel space, so use __GFP_HIGHMEM just like vmalloc
does. This patch reduces memory pressure slightly because pages can be
allocated in the high zone.

Fix a boundary condition that caused failure for certain device sizes.
The problem is reported at
http://code.google.com/p/cryptsetup/issues/detail?id=160
For certain device sizes the number of hashes at a specific level was
calculated incorrectly.
It happens for example for a device with data and metadata block size 4096
that has 16385 blocks and algorithm sha256.
The user can test if he is affected by this bug by running the
"veritysetup verify" command and also by activating the dm-verity kernel
driver and reading the whole block device. If it passes without an error,
then the user is not affected.
The condition for the bug is:
Split the total number of data blocks (data_block_bits) into bit strings,
each string has hash_per_block_bits bits. hash_per_block_bits is
rounddown(log2(metadata_block_size/hash_digest_size)). Equivalently, you
can say that you convert data_blocks_bits to 2^hash_per_block_bits base.
If there some zero bit string below the most significant bit string and at
least one bit below this zero bit string is set, then the bug happens.
The same bug exists in the userspace veritysetup tool, so you must use
fixed veritysetup too if you want to use devices that are affected by
this boundary condition.

Set noio flag while calling __vmalloc() because it doesn't fully respect
gfp flags to avoid a possible deadlock (see commit
502624bdad).
This should be backported to stable kernels 3.8 and newer. The kernel 3.8
doesn't have memalloc_noio_save(), so we should set and restore process
flag PF_MEMALLOC instead.

When multipath needs to retry an ioctl the reference to the
current live table needs to be dropped. Otherwise a deadlock
occurs when all paths are down:
- dm_blk_ioctl takes a reference to the current table
and spins in multipath_ioctl().
- A new table is being loaded, but upon resume the process
hangs in dm_table_destroy() waiting for references to
drop to zero.

Intel Smart Connect is an Intel-specific ACPI interface for configuring
devices to wake up at regular intervals so they can pull down mail or other
internet updates, and then go to sleep again. If a user enables this in
Windows and then reboots into Linux, the device may wake up if it's put to
sleep. Since there's no Linux userland support for any of this, the machine
will then remain awake until something else puts it back to sleep.
I haven't figured out all that much about how this works (there's a bunch
of different ACPI calls available on the device), but this seems to be
enough to turn it off. We can add more features to this driver if anyone
ever cares about figuring out what the rest of the calls do or writing some
Linux userspace to implement the rest of it.

Intel Rapid Start Technology is a firmware-based suspend-to-disk
implementation. Once placed in S3, the device will wake once either a
timeout elapses or the battery reaches a critical level. It will then resume
to the firmware and copy the contents of RAM to a specialised partition, and
then power off the machine. If the user turns the machine back on the
firmware will copy the contents of the partition back to RAM and then resume
from S3 as normal.
This driver provides an interface for configuring the wakeup events and
timeout. It still requires firmware support and an appropriate suspend
partition.

Controlling the wlan led seems to have some side effects to the machines
with wapf value is not equal to 4. It will make the keyboard backlight
out of order.
So, the patch will enable the wlan led function only if the wapf == 4.

Jon Bloomfield came up with a plausible explanation and cheap fix
(drm/i915: Fix incoherence with fence updates on Sandybridge+) for the
race condition, so lets run with it.
This is a candidate for stable as the old workaround incurs a
significant cost (calling wbinvd on all CPUs before performing the
register write) for some workloads as noted by Carsten Emde.

drm/i915: Workaround incoherence between fences and LLC across multiple CPUs
Thanks to further investigation by Jon Bloomfield, he realised that
the 64-bit register might be broken up by the hardware into two 32-bit
writes (a problem we have encountered elsewhere). This non-atomicity
would then cause an issue where a second thread would see an
intermediate register state (new high dword, old low dword), and this
register would randomly be used in preference to its own thread register.
This would cause the second thread to read from and write into a fairly
random tiled location. Breaking the operation into 3 explicit 32-bit
updates (first disable the fence, poke the upper bits, then poke the lower
bits and enable) ensures that, given proper serialisation between the
32-bit register write and the memory transfer, that the fence value is
always consistent.
Armed with this knowledge, we can explain how the previous workaround
work. The key to the corruption is that a second thread sees an
erroneous fence register that conflicts and overrides its own. By
serialising the fence update across all CPUs, we have a small window
where no GTT access is occurring and so hide the potential corruption.
This also leads to the conclusion that the earlier workaround was
incomplete.
v2: Be overly paranoid about the order in which fence updates become
visible to the GPU to make really sure that we turn the fence off before
doing the update, and then only switch the fence on afterwards.

Daniel noticed a problem where is we wrote to an object with ring A in
the middle of a very long running batch, then executed a quick batch on
ring B before a batch that reads from the same object, its obj->ring would
now point to ring B, but its last_write_seqno would be still relative to
ring A. This would allow for the user to read from the object before the
GPU had completed the write, as set_domain would only check that ring B
had passed the last_write_seqno.
To fix this simply (and inelegantly), we bump the last_write_seqno when
switching rings so that the last_write_seqno is always relative to the
current obj->ring.
This fixes igt/tests/gem_write_read_ring_switch.

This patch partially reverts commit 36ec8f8774 for
IvyBridge CPUs.
The original commit results in repeated 'Timed out waiting for forcewake old
ack to clear' messages on a Supermicro C7H61 board (BIOS version 2.00 and 2.00b)
with i7-3770K CPU. It ultimately results in a hangup if the system is highly
loaded. Reverting the commit for IvyBridge CPUs fixes the issue.
Issue a warning if the CPU is IvyBridge and mt forcewake is disabled, since
this condition can result in secondary issues.

It has been several years since the SCSI constants.c
file has been updated. The attached is against lk 3.10
and brings the command strings, sense keys and additional
sense code strings into sync with spc4r36g.pdf.
Certain SCSI command names that previously only took the
opcode (i.e. byte 0 of the cdb) into account, have been
split into several command names using the associated
service action field to differentiate. For example,
persistent reservations that previously had 2 commands
(i.e. "in" and "out") have been expanded to 12 commands
(e.g. "Persistent reserve in, read reservation").
Sync SCSI command names, sense key strings and additional
sense code strings with SPC-4 draft revision 36g

The LMMIO length reported by PAT and the length given by the LBA MASK
register are not consistent. This leads e.g. to a not-working ATI FireGL
card with the radeon DRM driver since the memory can't be mapped.
Fix this by correctly adjusting the resource sizes.

rtnl_is_locked() doesn't check who holds this lock, it just tells that it's
locked right now. if caif::ldisc_close really can be called under rtrnl_lock
then it should release net device in other context because there is no way
to grab rtnl_lock without deadlock.
This patch adds work which releases these devices. Also this patch fixes calling
dev_close/unregister_netdevice without rtnl_lock from caif_ser_exit().

vhost_net_ubuf_put_and_wait has a confusing name:
it will actually also free it's argument.
Thus since commit 1280c27f8e
"vhost-net: flush outstanding DMAs on memory change"
vhost_net_flush tries to use the argument after passing it
to vhost_net_ubuf_put_and_wait, this results
in use after free.
To fix, don't free the argument in vhost_net_ubuf_put_and_wait,
add an new API for callers that want to free ubufs.

The port number is only local to the ethernet block, not global, so
there can be two ethernet blocks both using the same port, like
kirkwood with both using port 0.
Fix this by using the array index offset for the allocated platform
devices as the id.

virtio net called virtqueue_enable_cq on RX path after napi_complete, so
with NAPI_STATE_SCHED clear - outside the implicit napi lock.
This violates the requirement to synchronize virtqueue_enable_cq wrt
virtqueue_add_buf. In particular, used event can move backwards,
causing us to lose interrupts.
In a debug build, this can trigger panic within START_USE.
Jason Wang reports that he can trigger the races artificially,
by adding udelay() in virtqueue_enable_cb() after virtio_mb().
However, we must call napi_complete to clear NAPI_STATE_SCHED before
polling the virtqueue for used buffers, otherwise napi_schedule_prep in
a callback will fail, causing us to lose RX events.
To fix, call virtqueue_enable_cb_prepare with NAPI_STATE_SCHED
set (under napi lock), later call virtqueue_poll with
NAPI_STATE_SCHED clear (outside the lock).

This adds a way to check ring empty state after enable_cb outside any
locks. Will be used by virtio_net.
Note: there's room for more optimization: caller is likely to have a
memory barrier already, which means we might be able to get rid of a
barrier here. Deferring this optimization until we do some
benchmarking.

ctlr_dev was initialized to NULL, and never re-assigned. This
caused the log statement to always report failure. This patch
removes the unused variable and fixes the log statement to always
report 'success', as that is what should be logged if the code
reaches this point.

Both fcoe_fc_els_lesb and fc_els_lesb are in __be32 already, and both are
exactly the same size in bytes, with somewhat different member names to
reflect the fact the former is for Ethernet media the latter is for Fiber
Channel, so, remove conversion and use __be32 directly. This fixes the warning
from sparse check.

do_device_access() is a function that abstracts copying SG list from/to
ramdisk storage (fake_storep).
It must deal with the ranges exceeding actual fake_storep size, because
such ranges are valid if virtual_gb is set greater than zero, and they
should be treated as fake_storep is repeatedly mirrored up to virtual
size.
Unfortunately, it can't deal with the range which wraps around the end of
fake_storep. A wrap around range is copied by two
sg_copy_{from,to}_buffer() calls, but sg_copy_{from,to}_buffer() can't
copy from/to in the middle of SG list, therefore the second call can't
copy correctly.
This fixes it by using sg_pcopy_{from,to}_buffer() that can copy from/to
the middle of SG list.
This also simplifies the assignment of sdb->resid in
fill_from_dev_buffer(). Because fill_from_dev_buffer() is now only called
once per command execution cycle. So it is not necessary to take care to
decrease sdb->resid if fill_from_dev_buffer() is called more than once.
