Since early 2.4.x all cdrom drivers implement the block_device methods
themselves, so they can handle additional ioctls directly instead of going
through the cdrom layer.

change it to CD_OPEN so it's supressed by default.

Change the blk_rq_map_user() and blk_rq_map_kern() interface to require
a previously allocated request to be passed in. This is both more efficient
for multiple iterations of mapping data to the same request, and it is also
a much nicer API.

Only issue a cdrom cache flush if we've done write to the drive. The
->media_written() flag keeps track of that.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
