b6a27d1e6b iwlwifi regression in 20111205 merge
It looks like the regression was introduced between 20111202 and
20111205 (linux-next tree). Symptoms: connection to AP seem to be
established, but no data goes though it in any way. Tested on intel
5300.
  Peek at the changes have shown that it looks like at least part of
the code wasn't merged properly. It was originally committed into
iwl_agn.c but code in question was moved to iwl-mac80211.c.
  This patch puts code in place and my card works again.

7185bb335a bnx2x: properly initialize L5 features
The code is missing initialization of NO_FCOE_FLAG and NO_ISCSI*FLAGS
when CONFIG_CNIC is not selected.
This causes panic during driver load since commit
1d187b34da where NO_FCOE tested
unconditionally (outside #ifdef BCM_CNIC  structure) and
accessed fp[FCOE_IDX] which is not allocated.

57459185a1 team: use vlan_vids_[addr/del]_by_dev
So far when vlan id was added to team device befor port was added, this
vid was not added to port's vlan filter. Also after removal, vid stayed
in port device's vlan filter. Benefit of new vlan functions to handle
this work.

8e586137e6 net: make vlan ndo_vlan_rx_[add/kill]_vid return error value
Let caller know the result of adding/removing vlan id to/from vlan
filter.

In some drivers I make those functions to just return 0. But in those
where there is able to see if hw setup went correctly, return value is
set appropriately.

e163cc97f9 net/fec: fix the .remove code
The .remove code is broken in several ways.
 - mdiobus_unregister() is called twice for the same object in case of dual FEC
 - phy_disconnect() is being called when the PHY is already disconnected
 - the requested IRQ(s) are not freed
 - fec_stop() is being called with the inteface already stopped

 All of those lead to kernel crashes if the remove function is actually used.

589efdc7f7 net/fec: don't munge MAC address from platform data
When the MAC address is supplied via platform_data it should be OK as
it is and should not be modified in case of a dual FEC setup.
Also copying the MAC from platform_data to the single 'macaddr'
variable will overwrite the MAC for the first interface in case of a
dual FEC setup.

9a215e40d7 iwlagn: fix TID use bug
The driver everywhere uses max TID count as 9,
which is wrong, it should be 8.

I think the reason it uses 9 here is off-by-one
confusion by whoever wrote this. We do use the
value IWL_MAX_TID_COUNT for "not QoS/no TID"
but that is completely correct even if it is 8
and not 9 since 0-7 are only valid.

As a side effect, this fixes the following bug:

 Open BA session requested for 00:23:cd:16:8a:7e tid 8
 ------------[ cut here ]------------
 kernel BUG at drivers/net/wireless/iwlwifi/iwl-trans-pcie-int.h:350!
 ...

when you do

        ssb: fix init regression of hostmode PCI core

In ssb_pcicore_fix_sprom_core_index() the sprom on the PCI core is
accessed, but the sprom only exists when the ssb bus is connected over
a PCI bus to the rest of the system and not when the SSB Bus is the
main system bus. SoCs sometimes have a PCI host controller and there
this code will not be executed, but there are some old SoCs with an PCI
controller in client mode around and ssb_pcicore_fix_sprom_core_index()
should not be called on these devices too. The PCI controller on these
devices are unused, but without this fix it results in an Data bus
error when it gets initialized.

The user can pass broadcast SSID (ssid="") in the list of SSIDs for active scan.
In this case the loop was attempting to match SSIDs in the filter

Enable ANI for ar9100 since it seems to be working fine (and as a
matter of fact ANI was always performed for ar9100 since code which
was supposed to disable it didn't achieve this goal).
  This patch sets config.enable_ani to default (true) value for

4279425cef ath9k: use config.enable_ani to check if ani should be performed
Currently in ath9k code there is an attempt which is meant to
disable ANI for ar9100 and ar9340. But it doesn't really achieve
this. All it does is disable ANI init and setup (i.e. calls to
ath9k_hw_ani_setup and ath9k_hw_ani_init). Since ath9k_hw_ani_setup is
not called ah->config.ani_poll_interval is never initialized (i.e. it
is always zero) and ath_ani_calibrate always executes ANI procedures
(over uninitialized ANI parameters).
  Moreover, ath_ani_calibrate is being called each 1ms because
common->ani.timer is set to zero interval because
ah->config.ani_poll_interval==0 (and thus smallest value of all
intervals). Normally it should not be called this often.
  This patch changes the code so config.enable_ani is used to check if
ANI should be performed.
  config.enable_ani is initialized to true by default. This patch sets
it to false for ar9100 and ar9340.

9df0d6a20a ath9k: only drop packets in drv_flush when asked to
Recently more places in mac80211 call drv_flush to ensure proper order
for state changes wrt. powersave, channel changes, etc. On some systems
such calls lead to spurious logspam about failing to stop tx dma, as well
as hardware resets that go along with that.
Instead of dropping packets in a place where it's completely unnecessary,
only do it when drop == true.

c0c1174144 ath9k: rework power state handling
Turning off the radio when mac80211 tells the driver that it's idle is not
a good idea, as idle interfaces might still occasionally scan or send packets.
The only time the radio can be safely turned off is when drv_stop has been
called. In the mean time, use sc->ps_idle only to indicate network sleep vs
full sleep.
Move the LED GPIO changes out of the PCI suspend/resume path, the start/stop
functions already take care of that.

c8e8868e3b ath9k: always issue a full hw reset after waking up from full-sleep mode
After waking up from full sleep, registers are accessible, but rx/tx
typically fails. A fast channel change will not recover from this, so
ensure that a full-sleep -> wake transition is always followed by a full
reset.

The reason why this hasn't created any serious problems yet is that it's
hidden by the (wrong) behavior of enabling/disabling the radio when the
wiphy idle state changes.

28d8ea2d56 fsl_pq_mdio: Clean up tbi address configuration
The code for setting the address of the internal TBI PHY was
convoluted enough without a maze of ifdefs. Clean it up a bit
so we allow the logic to fail down to -ENODEV at the end of
the if/else ladder, rather than using ifdefs to repeat the same
failure code over and over.

Also, remove the support for the auto-configuration. I'm not aware of
anyone using it, and it ends up using the bus mutex before it's been
initialized.

43af940c54 net/fec: fix the use of pdev->id
The pdev->id is used in several places for different purpose.  All
these uses assume it's always the id of fec device which is >= 0.
However this is only true for non-DT case.  When DT plays, pdev->id
is always -1, which will break these pdev->id users.

Instead of fixing all these users one by one, this patch introduces
a new member 'dev_id' to 'struct fec_enet_private' for holding the
correct fec device id, and replaces all the existing uses of pdev->id
with this dev_id.

1e226773c7 caif-hsi: Remove wake line modification when flushing FIFO
Raising wake before flushing FIFO and lowering it after caused a
spike on AC wake that were sometimes detected and acted upon by the

c7c2c39be8 r8169: fix Rx index race between FIFO overflow recovery and NAPI handler.
Since 92fc43b415, rtl8169_tx_timeout ends up
resetting Rx and Tx indexes and thus racing with the NAPI handler via
-> rtl8169_hw_reset
   -> rtl_hw_reset
      -> rtl8169_init_ring_indexes

What about returning to the original state ?

rtl_hw_reset is only used by rtl8169_hw_reset and rtl8169_init_one.

The latter does not need rtl8169_init_ring_indexes because the indexes
still contain their original values from the newly allocated network
device private data area (i.e. 0).

rtl8169_hw_reset is used by:
1. rtl8169_down
   Helper for rtl8169_close. rtl8169_open explicitely inits the indexes
   anyway.
2. rtl8169_pcierr_interrupt
   Indexes are set by rtl8169_reinit_task.
3. rtl8169_interrupt
   rtl8169_hw_reset is needed when the device goes down. See 1.
4. rtl_shutdown
   System shutdown handler. Indexes are irrelevant.
5. rtl8169_reset_task
   Indexes must be set before rtl_hw_start is called.
6. rtl8169_tx_timeout
   Indexes should not be set. This is the job of rtl8169_reset_task anyway.

The removal of rtl8169_hw_reset in rtl8169_tx_timeout and its move in
rtl8169_reset_task do not change the analysis.

811fd3010c r8169: Rx FIFO overflow fixes.
Realtek has specified that the post 8168c gigabit chips and the post
8105e fast ethernet chips recover automatically from a Rx FIFO overflow.
The driver does not need to clear the RxFIFOOver bit of IntrStatus and
it should rather avoid messing it.

The implementation deserves some explanation:
1. events outside of the intr_event bit mask are now ignored. It enforces
   a no-processing policy for the events that either should not be there
   or should be ignored.

2. RxFIFOOver was already ignored in rtl_cfg_infos[RTL_CFG_1] for the
   whole 8168 line of chips with two exceptions:
   - RTL_GIGA_MAC_VER_22 since b5ba6d12bd
     ("use RxFIFO overflow workaround for 8168c chipset.").
     This one should now be correctly handled.
   - RTL_GIGA_MAC_VER_11 (8168b) which requires a different Rx FIFO
     overflow processing.

   Though it does not conform to Realtek suggestion above, the updated
   driver includes no change for RTL_GIGA_MAC_VER_12 and RTL_GIGA_MAC_VER_17.
   Both are 8168b. RTL_GIGA_MAC_VER_12 is common and a bit old so I'd rather
   wait for experimental evidence that the change suggested by Realtek really
   helps or does not hurt in unexpected ways.

   Removed case statements in rtl8169_interrupt are only 8168 relevant.

3. RxFIFOOver is masked for post 8105e 810x chips, namely the sole 8105e
   (RTL_GIGA_MAC_VER_30) itself.

17e6abeec4 infiniband: ipoib: Sanitize neighbour handling in ipoib_main.c
Reduce the number of dst_get_neighbour_noref() calls within a single
call chain.  Primarily by passing the neighbour pointer down to the
helper functions.

Handle dst_get_neighbour_noref() returning NULL in ipoib_start_xmit()
by incrementing the dropped counter and freeing the packet.  We don't
want it to fall through into the ARP/RARP/multicast handling, since
that should only happen when skb_dst() is NULL.

11948b9335 toshiba_acpi: Fix machines that don't support HCI_SYSTEM_EVENT
The Satellite C670-10V generates notifications for hotkeys but does
not support HCI_SYSTEM_EVENT. As a result when a hotkey is pressed
it gets stuck in an infinite loop in toshiba_acpi_notify. To fix
this, detect whether or not HCI_SYSTEM_EVENT is supported up-front
and don't try to read system events if it isn't supported. In
addition, limit the number of retries when reading HCI_SYSTEM_EVENT
fails so that this loop cannot run unbounded.

43497cc218 ixgbe: DCB: IEEE transitions may fail to reprogram hardware.
Transitioning through an IEEE DCBX version from a CEE DCBX
and back (CEE->IEEE->CEE) may leave IEEE attributes programmed
in the hardware. DCB uses a bit field in the set routines to
determine which attributes PG, PFC, APP need to be reprogrammed.
This is needed because user flow allows queueing a series
of changes and then reprogramming the hardware with the
entire set in one operation.

When transitioning from IEEE DCBX mode back into CEE DCBX
mode the PG and PFC bits need to be set so the possibly

different CEE attributes get programmed into the device.

This patch fixes broken logic that was evaluating to 0
and never setting any bits. Further this removes some
checks for num_tc in set routines. This logic only worked
when the number of traffic classes and user priorities
were equal. This is no longer the case for X540 devices.
Besides we can trust user input in this case if the
device is incorrectly configured the DCB bandwidths will
be incorrectly mapped but no OOPs, BUG, or hardware
failure will occur.

4909fe979c ixgbe: DCBnl set_all, order of operations fix
The order of operations is important in DCBnl set_all(). When FCoE
is configured it uses the up2tc map to learn which queues to configure
the hardware offloads on. Therefore we need to setup the map before
configuring FCoE.

This is only seen when the both up2tc mappings and APP info are
configured simultaneously.

5f4a780ddd e1000e: hitting BUG_ON() from napi_enable
Based on a patch from Mike McElroy created against the out-of-tree e1000e
driver:

Hitting the BUG_ON in napi_enable(). Code inspection shows that this can
only be triggered by calling napi_enable() twice without an intervening
napi_disable().

I saw the following sequence of events in the stack trace:

1) We simulated a cable pull using an Extreme switch.
2) e1000_tx_timeout() was entered.
3) e1000_reset_task() was called. Saw the message from e_err() in the
console log.
4) e1000_reinit_locked was called. This function calls e1000_down() and
e1000_up(). These functions call napi_disable() and napi_enable()
respectively.
5) Then on another thread, a monitor task saw carrier was down and executed
'ip set link down' and 'ip set link up' commands.
6) Saw the '_E1000_RESETTING'warning fron the e1000_close function.
7) Either the e1000_open() executed between the e1000_down() and e1000_up()
calls in step 4 or the e1000_open() call executed after the e1000_up()
call.  In either case, napi_enable() is called twice which triggers the
BUG_ON.

9995ffe5f5 iwlwifi: change the default behavior of watchdog timer
The current default watchdog timer is enabled, but we are seeing issues on
legacy devices. So change the default setting of watchdog timer to per
device based. But user still can use the "wd_disable" module parameter
to overwrite the system setting

34a5b4b6af iwlwifi: do not re-configure HT40 after associated
The ht40 setting should not change after association unless channel switch

This fix a problem we are seeing which cause uCode assert because driver
sending invalid information and make uCode confuse

274b89ca3b iwlagn: fix HW crypto for TX-only keys
Group keys in IBSS or AP mode are not programmed
into the device since we give the key to it with
every TX packet. However, we do need mac80211 to
create the MMIC & PN in all cases. Move the code
around to set the key flags all the time. We set
them even when the key is removed again but that
is obviously harmless.

bab9efc206 vmwgfx: integer overflow in vmw_kms_update_layout_ioctl()
There are two issues in vmw_kms_update_layout_ioctl().  First, the
for loop forgets to index rects and only checks the first element.
Second, there is a potential integer overflow if userspace passes
in a large arg->num_outputs.  The call to kzalloc() would allocate
a small buffer, leading to out-of-bounds read.

6eebd6bb5f drm: Fix lack of CRTC disable for drm_crtc_helper_set_config(.fb=NULL)
Disabling the CRTC by setting its framebuffer to NULL, as used by
drm_framebuffer_cleanup(), was failing to pass the current framebuffer
to the crtc_func->disable callback. This is because of the dance within
drm_crtc_helper_set_config to pass the new_fb (NULL in this case) to the
drm_crtc_helper_set_mode with the currently attached fb as a parameter.
drm_crtc_helper_set_mode treats this as a no-op and the encoder is still
enabled. And so the current fb is forgotten before the call to
drm_helper_disable_unused_functions.

This patch treats disabling the CRTC as a simple special case rather
than adding further complexity into the configuration logic.

This fixes a pin-leak of the fb bo on Xserver close.

0de13500ad Revert "xHCI: reset-on-resume quirk for NEC uPD720200"
This reverts commit df711fc996.

The commit added a reset-on-resume quirk because the NEC chipset stopped
responding to commands about 30 minutes after a system resume from
suspend.  We thought it was a chipset issue, but it turns out that the
xHCI driver was zeroing out the link TRB after a successful context
restore during resume.  The host controller would fall off the command
ring sometime later, causing it to not respond to new commands.

The link TRB issue has been fixed with commit
158886cd2c "xHCI: fix bug in
xhci_clear_command_ring()", so revert the reset-on-resume quirk, as it's
not necessary.

Commit df711fc996 was marked for stable
trees back to 2.6.37, but according to my mail, it has not made it into
Linus' tree or the stable trees yet.

158886cd2c xHCI: fix bug in xhci_clear_command_ring()
When system enters suspend, xHCI driver clears command ring by writing zero
to all the TRBs. However, this also writes zero to the Link TRB, and the ring
is mangled. This may cause driver accesses wrong memory address and the
result is unpredicted.

When clear the command ring, keep the last Link TRB intact, only clear its
cycle bit. This should fix the "command ring full" issue reported by Oliver
Neukum.

This should be backported to stable kernels as old as 2.6.37, since the
commit 89821320 "xhci: Fix command ring replay after resume" is merged.

This patch adds support for legacy Bosch CC770 and Intel AN82527 CAN
controllers on the ISA or PC-104 bus. The I/O port or memory address
and the IRQ number must be specified via module parameters:

  insmod cc770_isa.ko port=0x310,0x380 irq=7,11

for ISA devices using I/O ports or:

  insmod cc770_isa.ko mem=0xd1000,0xd1000 irq=7,11

for memory mapped ISA devices.

Indirect access via address and data port is supported as well:

  insmod cc770_isa.ko port=0x310,0x380 indirect=1 irq=7,11

Furthermore, the following mode parameter can be defined:

  clk: External oscillator clock frequency (default=16000000 [16 MHz])
  cir: CPU interface register (default=0x40 [DSC])
  bcr: Bus configuration register (default=0x40 [CBY])
  cor: Clockout register (default=0x00)

Note: for clk, cir, bcr and cor, the first argument re-defines the
default for all other devices, e.g.:

  insmod cc770_isa.ko mem=0xd1000,0xd1000 irq=7,11 clk=24000000

is equivalent to

  insmod cc770_isa.ko mem=0xd1000,0xd1000 irq=7,11 clk=24000000,24000000

d7d75960ea icplus: mdio_write(), remove unnecessary for loop
At this point the variable j is always set to 7 and the code within
the loop has to run only once anyway.

As suggested by David Miller:
"You can simply this even further since p[7] is what is used here,
and this means len is one, the inner loop therefore executes only
once, and the p[7].field value is not used (it's zero in the table)
and the write to it is completely thrown away."

Use an appropriate mac80211 flags in CCMP keys to indicate we are
calculating the CCMP IV in HW, but require room for the IV to be reserved
in the skb. The space is reserved by mac80211.

341f2c11b5 wl12xx: avoid bail out when probe-resp is not set by mac80211
During reconfig we can get the BSS_CHANGED_AP_PROBE_RESP indication
even if a probe-resp has not been set in the first place. Therefore
ignore the error when not getting a probe-resp from mac80211. Resort to
the legacy probe-resp in this case.

Also take this opportunity to add a vif argument to the set_probe_resp
function.

If a driver wants to do command mode IO while CIO is doing
online path verification we ignore this request and provide
a fake irb when we are done and the driver can do IO again.

For transport mode IO we have no such mechanism, giving the
driver no other chance then to retry the action until we are
done. This is not very reliable.

Provide a fake irb for transport mode IO as well.

659213b899 [S390] cio: disallow driver io for known to be broken paths
When a driver requests to do IO, we will adjust the mask of
paths to be used to exclude varied offline paths.

Drivers trying to do IO solely on paths which are online but some
way defective may lack the information to do proper error handling.

There is no reason to allow the usage of known to be broken paths.
Thus restrict the paths a ccw driver can use for IO to a subset of
the paths cio found usable (this also excludes offline paths).

Use the proper macro to issue the debugging message in
radeon_atif_call(). Otherwise we spam the log of many systems with a
message which looks like an error message of unknown origin, and could
