9f5314fb4d x86, uv: update macros used by UV platform 
Update the UV address macros to better describe the
fields of UV physical addresses. Improve comments
in the header files. Add additional MMR definitions.

8400def825 x86: define the macros and tables for blade functions 
Add UV macros for converting between cpu numbers, blade numbers
and node numbers. Note that these are used ONLY within x86_64 UV
modules, and are not for general kernel use.
