Martin Schwidefsky e21ea246bc mm: remove ptep_test_and_clear_dirty and ptep_clear_flush_dirty
Nobody is using ptep_test_and_clear_dirty and ptep_clear_flush_dirty.  Remove
the functions from all architectures.
